Disability Benefits

Team members working a .5 FT or higher are eligible to participate in the Short and Long-Term Disability plan, subject to all terms and conditions of the agreement between Southwest Care Center and the insurance carrier. These plans are paid by Southwest Care Center on behalf of the team member with no financial obligation to the team member.

Team members will be required to submit medical certification as requested by the insurance carrier.  Required medical certification under this policy may differ from the medical certification required for any leave of absence requested including Family Medical Leave.

This is solely a monetary benefit and not a leave of absence. Team members who will be out of work must also request a formal leave of absence.
