
Disability Benefits
At no cost to the team member, SCC provides you with both a Short and Long-Term Disability Insurance plan through Mutual of Omaha. Disability insurance provides benefits that replace 60% of normal wages after the employee has been unable to work due to a covered injury or illness with a max cap of $1,500.00 per week. This benefit is available to all employees working .5 FTE or higher.
Benefits begin after 2 weeks of missed work. As soon as it is known that the need for Disability Insurance exists, please notify a human resources representative and use the links to the right to start the claims process. Should an approved short-term injury/illness extend beyond 13 weeks, the coverage automatically moves into a long-term disability covered event. Team members will be required to submit medical certification as requested by the insurance carrier. Required medical certification under this policy may differ from the medical certification required for any leave of absence requested including Family Medical Leave.

Important: If your salary exceeds $50,000.00 per year, long-term disability insurance is a taxable fringe benefit. The premiums are considered imputed income to you and the IRS sets the value of the benefit by age. When this applies, you will see this as 'imputed income' on your pay statements.

​​​Submitting a Disability Claim
Before starting the claims process, please read the Disability Claims Submissions Guide found to the right. This guide will provide important information that you will need to include on you claims application (i.e., group # located top left of 1st page). Additionally, it will provide details regarding next steps and tips to help expedite the process and can be found on page 2.
Also to the right there is a link for a Release of Information Authorization. Please ensure you complete an authorization form to include Southwest Care Center, Mutual of Omaha, and any physicians/providers that you will be requesting a certification from.
